Stillpoint Wellness Memberships Terms

Welcome to your Wellness Membership!  We commend you for choosing to be proactive about your health care.  Our goal is to make your membership as life and health enhancing as possible.  Please let us know of anything that we can do to improve your experience.  This document provides information on how your wellness membership works:
  1. Additional Wellness visits beyond plan:  On any given month, you may find that you would like to receive more Wellness visits than your membership level provides.  For example, if you have a membership that includes 4 visits per month, but you normally come in on a Tuesday, and a given month has 5 Tuesdays, you will likely want to add another visit to your membership that month.  No problem!  We will simply charge you on the day of your 5th visit at the rate that your current plan would break down to per individual visit, and then charge your standard membership rate at the end of your billing cycle.  So, in the above example, the 4 visit per month membership is $195.  Thus, an additional visit would be charged at $48.75, and $195 would be charged at the end of your billing cycle for a total of $243.75 for the 5 visits that month.
  2. Full Service Visits:  All Wellness Members are eligible for a 10% discount on full service visits!  Depending upon your situation, your practitioner may suggest to you that you receive one or more full service treatments per month in addition to your standard wellness visits.  Or you may need more intensive treatment on a given month because of an illness, injury, or other situation.  In this case, you have two options:
    1. simply pay for your full service visit at the discounted rate for wellness members (10% discount) at the time of service, and continue using your Wellness membership as you normally would. Alternatively, 
    2. upgrade one or more of your Wellness visits to a full service visit for only $25 (limit twice per billing cycle and five times/year).
  3. No Rollover: In the event that you do not use all visits during a given billing cycle, there is no carry over or accumulation of Wellness visits for future months.  In essence, if you do not use them, you lose them.  However, if you notify us in advance, you may place a hold on your membership as detailed below.
  4. Putting a membership on hold: You may put your membership on hold for 1 billing cycle out of any 12 month period from your initiation date.
  5. Cancellation: You may cancel your membership at anytime.  If you cancel in the middle of a billing cycle you will be charged for the number of visits you have received at the standard Wellness visit rate of $65/visit.  For example, if you have an 8 visit/month plan at $345/month and have received 3 visits during that particular billing cycle, you will not be charged the standard $345 at the end of the month, but rather $195 (3x$65) upon terminating your membership and no additional charges will be assessed.
  6. Billing Date: Your credit card will be automatically charged on the last day of the month.  Prepayment for medical services is not allowed.  Therefore you will receiving your first month of wellness membership visits before your card is charged.  So, keep in mind that if you terminate you wellness membership, your card will be charged at the end of the month for the treatments received during that month.
  7. Billing Period: Your billing period is by calendar month, beginning on the 1st day of the month and ending on the last day of the month.
